Aching joints and morning stiffness: what it is and how to help your body
You get up in the morning and your body feels wooden. Your knees, fingers and shoulders ache, though nothing has happened. This is often put down to "age". But this kind of aching in the transition has a clear cause — and clear steps.
Why it hurts
Oestrogen acts throughout the musculoskeletal system — in bones, joints, ligaments and muscles. When there is less of it, you feel it everywhere at once. Joint aches and stiffness without swelling affect more than half of women in the transition. This is part of the transition, not "just age".
On top of that, falling oestrogen removes the natural "brake" on background inflammation — so joints, sleep, tiredness and mood often drag each other down.
Another common story at 50–60 is "frozen shoulder": the arm is hard to raise both by yourself and with someone's help. For this — see a physiotherapist or orthopaedic specialist.
Rest or move?
Move. Strength training and regular movement are the foundation for joints, muscles and bones.
What helps
Strength training 2–3 times a week, without pain. This is the foundation for joints, muscles and bones alike. Strength training combined with walking or other cardio is best. With a trainer, the protection for bones is noticeably greater, and under supervision even intense strength training is safe.
A little jumping and impact — if your joints allow. This kind of load supports the bones of the hip and spine best.
Balance and mobility. Balance exercises help prevent falls, and therefore fractures.
Protein at every meal. After 40, muscle responds less well to protein and exercise — you need more of both.
A Mediterranean style. Oily fish, olive oil, berries, vegetables and whole grains lower background inflammation; processed meat, sweet drinks and ultra-processed food drive it up.
About bones — the part that doesn't hurt
Bone is lost fastest from about a year before the last period to two years after, and there are no symptoms until a fracture. Indirect signs: you have become more than 4 cm shorter than when you were young, you have started to stoop, your grip has weakened. If you have risk factors — menopause before 45, a fracture from a minor injury, long-term steroid use, a parent with a hip fracture — it is worth discussing a bone density scan with your doctor now. If the risk is high, doctors have treatments that noticeably lower the risk of fractures.
What about supplements?
- Calcium — better from food: yoghurt, cheese, sardines with bones, tofu. Calcium from food is safe, while large supplement doses are debated.
- Vitamin D is fine even without a test.
- Collagen does not help muscles.
When to see a doctor without delay
- A joint is swollen, hot or red; small joints of the hands hurt on both sides; morning stiffness lasts longer than half an hour to an hour and eases with movement — see a rheumatologist: it is important not to miss the first months.
- Sudden back pain from an ordinary movement — a cough, turning over in bed — together with loss of height.
- Any fracture from a minor injury.
- Back pain that wakes you at night, together with a fever or unexplained weight loss.
- Numbness in the groin area, weakness in both legs, problems passing urine — urgently.
What to try this week
- Two strength sessions of 30–40 minutes on different days; the basics are squats, pulls and presses, without pain.
- A source of protein at every meal.
- Calcium from food: yoghurt, cheese, sardines with bones or tofu every day.
