Desire and intimacy in the transition: what changes and how to get comfort back

5 min read

Desire comes less often, intimacy is sometimes unpleasant or painful, and it feels awkward to talk about even with a doctor. You are not alone: this is one of the most common changes in the transition — and one of the most solvable.

What changes with desire

In the transition, spontaneous desire — the kind that arises "by itself" — often fades. This is not a fault. For many women desire is responsive: it comes during intimacy rather than before it — and that is a healthy pattern.

Desire is affected by more than hormones. The biggest obstacles are:

Dryness and discomfort: no need to put up with it

Dryness, burning, pain during intimacy, frequent urges or recurring cystitis affect half of women after menopause or more. Yet they are rarely talked about: only about half tell their doctor, and most do not know this can be dealt with.

It is important to know: unlike hot flashes, this does not go away on its own and tends to get worse over time. But it responds well to help. And one more thing: pain leads to avoiding intimacy, and avoidance leads to less desire. Removing the discomfort often means getting desire back too.

What you can start right away, without a prescription

A moisturiser and a lubricant are different things. They are often confused:

Regularity matters as much as the product itself: "expensive" does not mean "effective".

Pelvic floor exercises and regular intimacy or stimulation support blood flow and comfort.

Remove irritants: intimate soaps and gels, douching, scented products, tight synthetic underwear. Drink enough water, don't smoke.

What a doctor can offer

If a moisturiser and lubricant are not enough, it is worth talking to a gynaecologist. There are local treatments, including low-dose oestrogen: they act where they are applied, are barely absorbed into the blood and, according to many studies, noticeably reduce both dryness and urinary symptoms. There are non-hormonal options too. Which one suits you is for a doctor to decide.

The strength of this evidence varies: some are large reviews and meta-analyses, some are small single studies. The steps in the article are simple, low-risk self-care measures, not treatment, and they may not suit everyone.

This is general information, not a prescription. Tests and treatment are chosen by a doctor.
