Desire and erections after 40: what men need to know

5 min read

Desire comes less often. Erections aren't as reliable as they used to be. It's awkward to talk about even with a doctor, and many men quietly look for "something online". Yet this is one of those changes that says a lot about your health — and it responds well to help.

Desire

Desire is affected most by extra weight, short sleep and chronic stress. Testosterone plays a part too: fewer morning erections and a quieter desire are what's most reliably linked to it falling. But testosterone itself most often drops because of that same weight, sleep and stress.

Medicines matter too: some antidepressants, finasteride, spironolactone, opioids and some blood pressure tablets. Don't stop them on your own — discuss a switch with a doctor.

Erections — an early signal from the blood vessels

An erection depends on healthy blood vessels. The vessels of the penis are several times thinner than those of the heart, so the same vascular problem shows up here earlier — often 2–5 years before heart problems. It's not a verdict but a window: the body warns you in advance, while there's still a lot you can do.

Not everything is vascular. There are clues:

Often it's a mix. Erection problems are quite often the first sign of diabetes: in about one in eight men.

What's worth checking with a doctor

Blood pressure, blood sugar (glucose or HbA1c), lipids and morning testosterone. It's inexpensive and shows a lot. Testosterone therapy raises desire but barely improves the erection itself — so checking the blood vessels matters more.

What helps — both blood vessels and erections

Aerobic exercise. The best-proven lever for erections specifically: walking, running, cycling, swimming — regularly.

Your waist. Losing weight helps; the effect builds over months.

Don't smoke. The strongest lever for the blood vessels.

A Mediterranean style. Berries, citrus fruit, olive oil, fish, whole grains.

Alcohol in moderation. Heavy and binge drinking harms erections; after quitting it's reversible.

The pelvic floor. Pelvic floor exercises are a useful addition.

Honestly: lifestyle works slowly, about a year or two. With a doctor, results come faster.

What to avoid — this matters for safety

Clinics actively sell shockwave therapy: there is an effect, but it's modest and not for everyone. Decide with a urologist, not by the ads.

When to see a doctor — urgently

What to try this week

The strength of this evidence varies: some are large reviews and meta-analyses, some are small single studies. The steps in the article are simple, low-risk self-care measures, not treatment, and they may not suit everyone.

This is general information, not a prescription. Tests and treatment are chosen by a doctor.
