Sleep after 40: snoring, apnea and why the night matters so much
You seem to sleep enough, yet you wake up feeling wrecked. You're drowsy during the day and out of energy by evening. Your partner complains about snoring and sometimes says you "stop breathing". Sleep changes for many men after 40 — and often the cause is something that's easy to check and treat.
Sleep apnea — the most common missed cause
With apnea, the airway keeps collapsing during sleep: breathing stops, oxygen drops, and the brain wakes the body for a moment. You don't remember it in the morning, but the night is broken into pieces.
About a third of men over 40 have apnea, and even more of those carrying extra weight. In more than 80% of cases nobody knows. Signs:
- loud, habitual snoring;
- breathing pauses your partner notices;
- waking up feeling like you're choking;
- daytime sleepiness, especially behind the wheel;
- getting up to pee at night;
- morning headaches, sleep that doesn't refresh.
Apnea is not a minor thing. It worsens, and is worsened by, extra weight, high blood pressure, heart rhythm problems and low testosterone. The good news: it's treatable, and treatment works. The main difficulty isn't the treatment — it's that nobody looks for it.
What about watches and rings?
Oxygen dips, night-time heart rate, an apnea notification — these are a flag, a reason to discuss a sleep study. But no notification doesn't mean no apnea: watches miss the milder forms.
What works
A fixed wake-up time. All seven days, weekends included. Regular sleep matters as much as its length. Aim for 7–9 hours.
An evening without alcohol. It hits three times: it relaxes the throat muscles and worsens snoring and apnea, lowers testosterone, and cuts into the second half of the night. Even two glasses.
Coffee in the morning. Daytime coffee keeps stress hormones high and cuts deep sleep. The last cup — 8 or more hours before bed.
The bed is for sleep only. If you can't sleep for more than 15–20 minutes, get up and come back when you feel sleepy. This is the core of behavioural therapy for insomnia, and it's stronger than pills.
Losing weight. Minus 10–15% noticeably reduces how severe apnea is.
Don't sleep on your back if you snore more in that position.
If a sleep study shows apnea, the gold standard is a machine with a mask (CPAP). Honestly: it's good at bringing back alertness and safety at the wheel, and lowers blood pressure a little. But it only works if you use it every night. There are other options too — mouth guards, treating weight. You choose with a doctor.
What to avoid
- Sleeping pills "for insomnia" until apnea has been ruled out: they relax the throat and deepen the oxygen dips.
- "Sleep" supplements. Melatonin helps modestly, magnesium and glycine weakly, valerian has no evidence. We don't recommend ashwagandha: there are risks for the liver and the thyroid. No supplement treats apnea.
- Testosterone with untreated apnea. The hormone can make apnea worse. Sleep first, then a conversation about hormones.
When to see a doctor
- Loud snoring with breathing pauses and daytime sleepiness — discuss a sleep study.
- You fall asleep at the wheel — without delay.
- High blood pressure that's hard to bring down, or repeated rhythm problems.
- You get up to pee more than twice a night and you snore — it may be apnea, not the prostate.
What to try this week
- Get up at the same time all seven days.
- An evening without alcohol, coffee only in the morning.
- Ask your partner whether you have pauses in breathing during sleep.
